Our corrections policy explains how to report public-source issues and how we manually review updates without collecting private health information.
Corrections Policy
mychart-health.org/ is built around human review and public-source accuracy. If a public link, phone number, address, map reference, organization name, portal instruction, or safety notice is wrong or outdated, we want to review it and correct the page where confirmed.
What We Correct
- Broken or outdated official login links.
- Incorrect public patient support phone numbers.
- Outdated billing, pay-as-guest, estimate, or financial assistance page references.
- Incorrect address or map details shown on our informational pages.
- Confusing instructions that could send users to the wrong provider support team.
- Missing privacy, emergency, or account-safety warnings.
- Typos, formatting problems, and mobile readability issues.
What We Cannot Correct
We cannot correct medical records, portal account data, provider notes, test results, appointments, prescriptions, bills, insurance information, proxy access, or official healthcare organization records. Those issues must be handled by the official healthcare organization or provider support team.
How to Request a Correction
Review Process
Correction requests are manually reviewed. We compare the reported issue with public official sources where available. If confirmed, we may update a link, replace a phone number, clarify an office role, add a safety warning, rewrite a section, or remove outdated information. If the issue cannot be verified but could affect user safety, we may still make the wording more cautious.
Patient Record Issues
If your concern involves your personal medical record, billing account, appointment, prescription, test result, or portal access, contact the official healthcare organization directly. Our corrections policy applies only to public content on mychart-health.org/.
