Do not submit protected health information or sensitive account details through our contact methods. We do not need private patient information to review public website corrections.
Examples of Information Not to Send
- Medical record number, patient ID, or account number.
- Insurance member ID, group number, or claim details.
- Social Security number, date of birth, or identity documents.
- Test results, medications, diagnosis, visit summaries, clinical notes, or provider messages.
- Appointment letters, discharge summaries, referral documents, or prescription labels.
- MyChart username, password, activation code, verification code, or security answers.
- Billing statements, guarantor number, bank details, card details, or payment screenshots.
What You Can Send Instead
For a public website correction, send only the article URL, the organization name, the public official source link, and a general description such as “the support phone number on your guide appears outdated.” Keep your message about the public page, not your private health account.
HIPAA and Official Privacy Rights
HIPAA and healthcare privacy rights are handled by covered healthcare organizations and their official privacy officers, not by this informational website. If you need to request records, amend a record, ask about a privacy incident, file an authorization, revoke access, or ask about disclosure rights, contact the official healthcare organization directly.
Proxy and Caregiver Access Privacy
Proxy access for a child, spouse, parent, dependent, or caregiver is controlled by the healthcare organization. Rules may vary based on age, state law, relationship, guardianship, authorization forms, and patient consent. We can explain the general idea of proxy access, but we cannot approve it, deny it, or review private documents.
