Rady requires the process to start in person and be approved by the County Placement Worker and Health Information Management.

Adult designated caregiver

Something in the chart is wrong

MyChart displays information from the child’s electronic medical record. Rady says inaccurate clinical information should be discussed with the doctor so the source record can be corrected.

Preferred name vs legal name: Rady says preferred-name changes can be made from Personal Information in MyChart. A legal-name change requires an office visit with updated identification.

Use proxy access before messaging about another family member. A message becomes part of the chart from which it is sent. Do not place one child’s medical issue into another person’s record.

Appointments and EZ Arrival: Start 48 Hours Before the Visit

Rady calls its MyChart pre-check-in workflow EZ Arrival. Completing it before leaving home can prevent spending clinic time repeating insurance, medication and consent information.
