Before the transition
Loyola patients used the older myLoyola portal. Historical information from that system is not simply erased.
From February 28, 2026
New visits, prescription refills, visit notes and lab results are placed in the new Trinity Health MyChart environment.
Old information
Loyola says historical data will transition over time. The old patient portal remains available for historical information but becomes read-only; no new information is added there.

Loyola says existing myLoyola account holders can generally use their existing username and password with the new MyChart system.
Username conflict?
If your existing username is already being used by another account in the new system, Loyola says an activation code and instructions can be sent so you can create a new username.

At a visit, Trinity Health may send a MyChart instant-activation link by text or email. The current FAQ says this link is valid for 24 hours and does not require a separate activation code.

Loyola’s new MyChart has specific rules for adolescents. The transition FAQ says teen patients with an older myLoyola account must complete a new setup process after February 28, 2026.

Do not send a message about another person from your own chart. Trinity Health warns that doing so places the other person’s information in the wrong medical record. Switch to the authorized proxy chart first.
Spouses and adult relatives should not share one username/password. Each person keeps their own account and grants approved proxy access when needed.

Loyola’s online medical-record request application provides request-status tracking. The official records page tells patients to have a driver’s license or other photo ID ready. Records may be supplied through the MRO online portal, U.S. mail or secure email.
Before paying for a complete copy: check whether the results, notes or visit document you need can already be downloaded from MyChart. A smaller abstract may also satisfy many personal, school or continuity-of-care needs.
