UF Health’s patient portal is called MyUFHealth. UF Health’s current portal FAQ says MyUFHealth was formerly known as MyChart, which is why patients still commonly search for “UF Health MyChart,” “Shands MyChart” or “UF MyChart.”
The same MyUFHealth account can support care across participating UF Health services in Gainesville, Central Florida, Jacksonville and St. Johns. This guide covers the current login, account recovery, activation, MFA, 2026 passkey changes, app requirements, test results, formal records, family access, billing, telemedicine and region-specific support.
Quick answer: UF Health MyChart is now branded MyUFHealth. Use the official portal at mychart.shands.org. Existing patients can sign in, use account recovery, pay as a guest or request an estimate there. UF Health says MyUFHealth is available to patients seen in its Gainesville, Central Florida, Jacksonville and St. Johns networks.

“UF Health MyChart” and “MyUFHealth” Mean the Same UF Health Portal
UF Health’s current FAQ describes MyUFHealth as formerly known as MyChart. The login screen is still an Epic MyChart system and the mobile application patients use is Epic’s MyChart app, but UF Health’s patient-facing portal name is MyUFHealth.
Use MyUFHealth for UF Health care
The portal may contain appointments, messages, medications, refill requests, test results, billing tools and portions of your UF Health electronic medical record.
It is not the complete legal medical record
UF Health explicitly states that MyUFHealth provides access to portions of the medical record. If you need the complete record, use the formal Medical Records process described below.

MyUFHealth Signup: Age Rules, Activation Codes and Self-Signup
UF Health has two different age rules that are easy to confuse: patients can have their own MyUFHealth account starting at age 12, but online self-signup without an activation code is limited to current UF Health patients age 18 or older.
If you already have an activation code
UF Health says a code may arrive by email before an appointment or appear on a visit summary or discharge paperwork. Enter it only on the official MyUFHealth activation page.
- The code is not your permanent username.
- It expires after 90 days.
- It stops working after it has been used once.
- You create your own username and password during enrollment.
If you do not have an activation code
Current UF Health patients age 18 or older can use the self-signup option. UF Health says identity verification can be performed through a third-party verification system.
If you are age 12–17, use the account-enrollment process provided by your UF Health clinic rather than the adult self-signup route.
Signup error involving your ZIP code? UF Health’s current FAQ says an “invalid personal information” error can occur when the ZIP code entered does not match the ZIP code in the UF Health medical record. Ask your clinic to check the demographic information on file rather than repeatedly resubmitting different addresses.
How to Log In and Recover a MyUFHealth Account
Open the official UF Health portal
Confirm the browser is on mychart.shands.org. MyUFHealth requires JavaScript and cookies for normal login.
Use your current MyUFHealth username or configured sign-in method
The 2026 login page supports a MyUFHealth username/Epic ID and also shows passkey login for accounts that already have a passkey configured.
Complete multi-factor authentication
UF Health now requires MFA. Keep the security code private and enter it only on the official portal.
If credentials fail, use “Forgot login information?”
Enter the verified email address or phone number attached to the account, receive the code, verify it, then enter your date of birth. MyUFHealth can then display your username or let you continue to password reset.
Stop after repeated failed attempts
UF Health says more than five incorrect username/password attempts can deactivate the account. Once deactivated, the clinic or MyUFHealth support must reactivate it.
Automatic timeout: MyUFHealth currently logs users out after about 10 minutes of keyboard inactivity. If you step away and return to a login screen, that is usually the privacy timeout—not proof that your password changed.
2026 Security Update: MFA Is Required and Passkeys Are Available
UF Health’s current MyUFHealth FAQ says multi-factor authentication is required to help protect test results, appointment information and care-team messages even if someone learns your password.

- Keep your email address and phone information current.
- Do not give a code to anyone claiming to be portal support.
- If recovery fails, use the regional UF Health support number below.
Passkeys — available starting September 2, 2026
UF Health says passkeys can use a fingerprint, face or device PIN instead of a password. A passkey must first be created in a web browser.
- Sign in to MyUFHealth in a web browser.
- Open Menu.
- Open Account Settings.
- Choose Manage passkeys.
- Select Add new passkey.
- Follow the operating-system/browser prompts.
MyChart Mobile App: Important September 2026 Requirements
UF Health uses Epic’s official MyChart mobile app for MyUFHealth access. A current 2026 platform change is important if an older phone suddenly stops signing in.
Effective September 2, 2026: UF Health says users need MyChart mobile app version 11.4 or later to continue using the latest features and sign in reliably.

Use the same MyUFHealth credentials you use on the UF Health website. If the mobile app itself fails, UF Health’s FAQ advises contacting your clinic for help.
MyUFHealth Bedside: UF Health’s current portal terms say Bedside may be available during certain hospital or emergency department encounters. It can expose selected clinical information and communication tools during the stay, but it does not replace emergency communication with hospital staff.
When UF Health Test Results Appear in MyUFHealth
UF Health releases many results automatically, so a patient may see the result before the ordering clinician has reviewed it or added a comment.

Typical response time
UF Health says a message to a provider or support staff member should typically receive a response within about 2 business days.
Another authorized member of the clinic team may reply if the provider is unavailable.
Do not discuss another patient from your own chart
UF Health warns against sending questions about a child, spouse or family member through your own MyUFHealth account because portal communications are saved in your personal record. Use approved proxy access instead.
Do not use MyUFHealth for urgent medical needs. UF Health’s terms state that portal messages may take up to two business days. Call the physician’s office for urgent issues; call 911 or use emergency care for an emergency.
Request a Formal Medical Record Through MyUFHealth
You do not necessarily need to start with a paper authorization form. UF Health’s current FAQ provides a formal record-request workflow directly inside MyUFHealth.

- Under Sharing, choose Request formal copy of health record.
- Complete and submit the request form.
- Wait for Medical Records staff to release the requested material.
- Open Document Center → Requested Records to view/download it.
UF Health currently asks patients to allow up to 10 business days for formal record requests to be completed.
Formal records can include radiology images
UF Health’s FAQ says its formal MyUFHealth record workflow can be used to request and receive records including radiology images from Medical Records.
If you need the complete legal record rather than the portion shown in the portal, this is the safer route.
School Health Summary
Parents and patients may also find a simpler school-focused summary useful. Inside MyUFHealth go to: Menu → Sharing Hub → Download School Health Summary.
UF Health says this summary can include basic demographics, vital signs, allergies, current medications, immunizations, health issues and the current care-team list.
Need to correct the legal medical record? UF Health says amendment requests must be in writing, signed and dated, identify the records and facility involved and explain the requested correction. UF Health says it will respond within 60 days.
Family Access: Child, Teen and Adult Proxy Rules
Family access is not simply password sharing. UF Health provides proxy workflows so a parent, caregiver or trusted adult can use their own MyUFHealth login.

Timing: UF Health says online requests for child proxy access may take up to 5 business days. Some situations require in-person setup and photo identification.
If the intended adult proxy does not already have a MyUFHealth account, UF Health says the patient can visit the clinic and complete a consent form. The clinic can create a non-patient proxy account and send that person an activation code.
Appointments and Telemedicine: Check the Specialty Workflow
MyUFHealth can support appointment requests, scheduling and upcoming/past visit information, but not every appointment type is available for direct online booking.
Returning patients
Many UF Health clinics allow returning patients to request or schedule eligible follow-up care through MyUFHealth.
If the visit reason or provider is not available online, call the clinic rather than choosing an inaccurate appointment type.
New-patient scheduling
Some UF Health clinics direct new patients to phone scheduling before portal access is useful. For example, UF Health General Internal Medicine currently directs new patients to the Patient Access Center at 352-265-5555 while returning patients can use MyUFHealth.
UF Orthopaedics telemedicine example
UF Health Orthopaedics currently instructs scheduled telemedicine patients to prepare MyUFHealth/MyChart and Zoom before the visit. Its published workflow says that about 10 minutes before the appointment:
- Sign in to MyUFHealth on a computer, or MyChart on a mobile device.
- On desktop, open Visits → Appointments and Visits.
- Choose today’s visit.
- Select Begin Video Visit.
- On mobile, open the appointment and select Begin Visit.
- Make sure camera, microphone and speakers are working.
This is the published UF Orthopaedics workflow, not a promise that every UF Health specialty uses exactly the same telemedicine software or menu.
Billing: Use MyUFHealth, but Match the Number to the Bill
UF Health includes multiple hospitals, faculty practices and regional systems. A billing number that is correct for a Gainesville physician bill may not be the correct number for a Jacksonville or separate facility statement.

Best billing rule: use the phone number and entity name on the actual statement when there is any conflict. UF Health facilities and clinicians can generate different charges, and an Explanation of Benefits from your insurer is not the same thing as a bill.

UF Health says you can update email, password and demographic information from Menu → Personal Information. You can also submit requests to update allergies or medications.
Change your MyUFHealth username

Use a clear subject such as Username Change.
Account disabled after failed logins
