Upstate MyChart Signup: What the Current Form Asks For
Upstate lets patients receive an activation code through care or request an account online. Its current signup form explains that processing and verification can take up to one week when automatic verification is not completed immediately.

Two-Factor Authentication: Upstate’s October 18, 2026 Change
Upstate’s current security notice says two-factor authentication will be required for MyChart beginning October 18, 2026. Login will require the account username, password and a one-time verification code.

For new parents
Upstate says postpartum patients can see newborn-related information through Bedside during the hospital stay when the feature is available to the account.
2026 update: Upstate announced that beginning in March 2026, MyChart Bedside would display discharge-related care progression. Upstate also introduced inpatient-TV QR codes to help patients who had not yet created MyChart enroll during their stay.
Appointments: 2026 Reschedule, Cancel and Waitlist Expansion
Upstate expanded MyChart appointment self-service in July 2026. Some patients can now reschedule or cancel appointments and join a waitlist for an earlier opening directly through MyChart.
If a self-service button appears

- Open your upcoming appointment.
- Use the reschedule, cancel or waitlist option shown for that visit.
- Review the new date/time or cancellation status before leaving the screen.
If the option does not appear
Not every clinic and appointment type supports direct self-service. Upstate’s portal says unsupported appointments can still allow an appointment request, and the clinical office remains the fallback route.

Results can appear before the clinician calls
Upstate says most test results become available when released to MyChart. You may see the result before your provider has reviewed it or added interpretation.
Messages are not instant
Upstate’s MyChart FAQ says patients generally receive a reply within 1–3 business days. Its terms advise allowing at least three business days for electronic inquiries.
Update ordinary personal information
Upstate says the Personal Information page can be used to update items such as address, phone number, email address and preferred name.
Legal-name changes are different. Upstate’s FAQ says formal legal-name changes require legal proof such as an adoption document, court name-change document, birth certificate, qualifying driver’s-license documentation, marriage certificate, passport or certificate of naturalization.
Medical information incorrect? Upstate’s MyChart FAQ says information comes from the electronic medical record and recommends asking the provider to correct inaccurate clinical information. Do not use MyChart messages for an emergency. Upstate directs patients to the medical center for immediate concerns and to call 911 for an emergency.
Parent, Teen and Adult Proxy Access
Upstate’s proxy rules are age-sensitive. A child turning 12 changes what a parent or guardian can see through MyChart because New York privacy rules protect certain adolescent health information.

Upstate reminded employees in 2025 that staff Epic access must not be used to look at their own or family members’ records without a work-related purpose. Family access should go through MyChart proxy access, Health Information Management or a signed patient authorization.
