How to Log In to UT Southwestern MyChart Safely

Open the official UTSW MyChart portal

Confirm the browser address is on mychart.utsouthwestern.edu before entering credentials. The current page supports standard username/password login and can also show passkey login for accounts that have configured a passkey.

Enter your existing credentials

Use the account attached to your UT Southwestern patient record. Do not create another account simply because an old password no longer works.

Complete two-factor verification

UT Southwestern moved MyChart sign-ins to required two-factor authentication in 2025. After username/password verification, the portal can send a one-time code by text or email.

New UT Southwestern MyChart Account: What Happens Automatically

UT Southwestern’s current MyChart guidance distinguishes between new adult patients and established patients who have simply never activated portal access.

New adult patient age 18+

UT Southwestern says new patients age 18 or older who are under the care of a UTSW provider are automatically enrolled in MyChart.

Follow the enrollment or activation information sent through the official workflow rather than creating an unrelated patient account.

Existing UTSW patient without MyChart

UT Southwestern directs existing patients who are not already using MyChart to contact their provider’s clinic. Clinic staff can help establish the appropriate access.

Activation code expired? UT Southwestern says activation codes are single-use and expire after 60 days. Request fresh access through the official portal or clinic rather than continuing to enter an expired code.

MyChart Messages Can Be Free—or Become a Billable E-Visit

This is one of the most useful UTSW-specific details to understand before sending a medical question. UT Southwestern does not charge simply because you opened the Message Center, but some clinical responses can meet the criteria for a billable electronic visit.

Possible patient cost: UT Southwestern says insurance is billed first for qualifying e-visits and that deductibles, copays or coinsurance may apply. Its current patient guidance states a maximum patient charge of $50 even when insurance does not cover the service.

Write one clear question. UTSW currently limits a MyChart message to about 1,500 characters and says its clinics collectively receive thousands of MyChart messages each day. Concise messages are easier to route and answer.

Do not use MyChart for urgent or emergency symptoms. Messages are not continuously monitored at night, on weekends or holidays. UT Southwestern tells patients to allow up to 3 business days for a response. Call 911 for a medical emergency.

  1. The patient signs into their own MyChart account.
  2. Open Menu.
  3. Find Proxy Invite under the blue Sharing section.
  4. Select Invite Someone under “Who Can See My Record?”
  5. Enter the proxy’s information and accept the terms.
  6. Select Send Invite.
  7. The invited proxy follows the email invitation and verifies using the patient’s date of birth.

Minor or incapacitated-adult request

  1. The parent/legal representative signs into their own MyChart account.
  2. Open Menu → Proxy Invite.
  3. Select Request Proxy Access.
  4. Enter the patient information.
  5. Certify the legal right to access the record.

How to Request a Formal Copy of UT Southwestern Medical Records

Active MyChart users can request and receive a formal electronic copy through the portal. MyChart is the preferred route for many requests, but radiology images currently require a separate records workflow.
